從混合性研究中探討澳門長者對睡眠健康認知及其相關之因素

Abstract
背景:睡眠障礙在老年族群中盛行率極高,影響因素多且複雜,對生活影響和牽連性較廣,故睡眠品質已成為長者生活質量重要因素之一。目的:本研究探討長者如何理解和體驗睡眠,睡眠品質與生品質之關係。研究方法:本研究採以混合研究的方法,以匹茲堡睡眠品質量表來篩查有睡眠障礙困擾之長者,描述長者人口學特徵和睡眠品質狀況,再以半結構式訪談方法來收集睡眠障礙困擾之相關因子,應用質性研究中的現象學進行分析。
結果:本次研究共收集了 61 位參與者,平均年齡為 81.86 歲,女性居多,匹茲堡睡眠品質得分平均值為 8.93 分,顯示有 75.5%長者有睡眠不佳的情況。當中深度訪談了10位睡眠品質欠佳的長者,從個人經驗、心理方面、環境方面、生活型態、痛症、生理疾病和家庭方面與睡眠品質進行分析,而心理因素導致受訪者睡眠品質的主因。結論:主觀評估和自我報告會影響睡眠質量,而年齡是影響長者的睡眠質量因素之一。由此觀之,睡眠品質與生活之間有密切的關係,故提出有關睡眠健康行為和措施,有助於長者維持健康和提升睡眠品質。
Background: Sleep disorders have a high incidence in the older people population which seriously affect the quality of life and physical and mental health of elderly. Therefore, sleep quality has become one of main factors of the elderly's life quality. Objective: This study explores how the elderly understand and experience sleep and the relationship between sleep quality and health quality. Methods: This study was project to mixed-methods research, with Pittsburgh Sleep Quality Index (PSQI) screening with sleep disorders of the elderly, describe the elderly demographic characteristics and sleep quality status, and semi-structured interview method to collect the impact factors of sleep disorders, analyze the application of phenomenology of qualitative research. Results: A total of 61 participants were collected in this study, the average age was 81.86 years old, and most of them were female. The average score of PSQI was 8.93,
indicating that 75.5% of the elderly had poor sleep. Random sample of ten with poor sleep quality were deeply interviewed, and their sleep quality was analyzed from personal experience, psychological aspects, environmental aspects, lifestyle, pain, physical diseases and family aspects, while psychological factors were the main causes of sleep quality. Conclusion: subjective sleep measurement and self-reported sleep quality were affected sleep quality, and age is also ore of factors affecting sleep quality in the elderly. There is close relationship between sleep quality and quality of life Therefore, healthy sleep behaviors and measures are proposed to help the elderly maintain their health and improve sleep quality.
